The challenge

Care coordination crossed organizations, roles and systems with different permissions and responsibilities. Teams needed one dependable product without weakening privacy, accountability or clinical context.

We began by mapping the people, information, decisions and constraints behind the visible product request. That operating picture shaped both the experience and the technical boundaries.

The solution

We shaped a multi-tenant care platform around role-aware journeys, secure data boundaries and shared coordination workflows. The architecture supports clinical integrations while keeping everyday work clear for each participant.
